Use this form to tell us about you and your trip. We'll be able to confirm if you are covered and any additional premiums that may apply. In the event of a claim, we may require you to provide evidence that the information provided is correct. This form is for overseas travel insurance only.

Your Card, Dates and Region

If you are unsure, check your statement to confirm it is an ASB Visa Platinum Rewards card.

Other Approved Methods include using True Rewards Dollars, Airline frequent flyer points, such as Air New Zealand Dollars, using a cash advance from your Platinum card account, winning your tickets in a prize draw or using funds from Any ASB account (if credit card is not a valid means of payment, or if additional charges are apply to using your credit card).

We use this to verify your card type.

Do you hold a return ticket for your trip?

You must hold a return travel ticket to be eligible for cover. Separate one way bookings departing from and returning to New Zealand are considered to be return travel tickets, even if they are booked with separate providers.

This is the date you are scheduled to depart from New Zealand. If you are traveling domestically prior to your international departure, do not include domestic travel days. Please include the time of your scheduled travel so we can be certain of the number of days, measured in consecutive 24-hour blocks, that you are overseas (NZ time zone). Ensure you consider time differences/zones and travel times.

This is the date you are scheduled to arrive in New Zealand, not the date you board your flight. If you are traveling domestically following your arrival in New Zealand, do not include domestic travel days. Please include the time of your scheduled travel so we can be certain of the number of days, measured in consecutive 24-hour blocks, that you are overseas (NZ time zone). Ensure you consider time differences/zones and travel times.

To be eligible for complimentary cover your return trip must be within 90 days.

Card Holder

There is no automatic medical cover for persons aged 75 years and over. Information about age limits can be found here.

Are you a citizen or permanent resident of New Zealand?

Cover may be limited if you are not a New Zealand Citizen or permanent resident. 

This policy provides cover for cardholders intending to return to New Zealand at the conclusion of their travel. If you do not hold New Zealand citizenship or Permanent Resident status then AIG may not be able to assist with medical evacuation in the event of a medical emergency overseas. In the event that you cannot return to New Zealand to continue receiving treatment due to your residency status then cover under the policy may no longer be available. If you are unsure about your residency status, please seek independent advice. AIG cannot provide any advice relating to your immigration status.

Do you require medical cover for a pre-existing condition?

You are not automatically covered for pre-existing medical conditions. Information about pre-existing medical conditions can be found here.

Are you on a waiting list for treatment in a hospital?

You cannot apply to cover pre-existing medical conditions if you are on a waiting list for in-patient treatment for one or more conditions.

Are you travelling overseas to seek medical treatment or advice?

You cannot apply to cover pre-existing medical conditions if part of your trip is for medical treatment or advice. This includes cosmetic or dental surgery.

Have you been diagnosed with a terminal condition?

You cannot apply to cover pre-existing medical conditions if you have received a terminal diagnosis.

Do you have symptoms without a known diagnosis or for which you are awaiting the result of diagnostic tests?

You cannot apply to cover pre-existing medical conditions if you do not have a confirmed diagnosis. You can return to this site later and complete an application for a cover extension if you subsequently receive a confirmed diagnosis. Tests to monitor the status of a diagnosed condition are not considered to be diagnostic tests.

Will your Spouse be accompanying you for the entire trip?

Spouse means: A person to whom you are legally married, or with whom the ASB Visa Platinum Rewards Cardholder lives in a relationship equivalent to marriage (whether of the same or opposite gender) and has done so for a continuous period of at least 3 months prior to the date Travel commences.

Accompanying means departing with you, returning with you and following the same itinerary. This allows for side trips of up to 5 days or 20% of your trip, whichever is the lesser.

Will dependent children be accompanying you for the entire trip?

Dependent Child(ren): means, a person who

• is the natural, step, or legally adopted child of the ASB Visa Platinum Rewards Cardholder; and
• is unmarried and not living in a relationship equivalent to marriage; and
    - is under the age of 18 years at the date Travel commences; or
    - is a full-time student at an accredited institution of higher learning and is primarily dependent on the ASB Visa Platinum Rewards Cardholder for maintenance and support and is under the age of 25 years.

Optional Cover
Do you need cover for high value personal items?

Cover for any item, pair or set of items is subject to a limit of $2,000 (except for laptop computers (inclusive of all accessories), which is $6,000). Information about high value items can be found here.

Include make or model information if available. You will be required to supply proof of ownership and valuation of the item in the event of a claim. Note that sporting equipment cannot be covered as high value items.
